HRT alternatives

by Dr Sarah Brewer

question
Last year, I had a complete hysterectomy. But afterwards, I was put on hormone replacement therapy and it's causing problems. I feel like I don't know who I am anymore. Do I have to take it? What happens if I don't? I have also lost all interest in sex.

answer
If you stop the HRT suddenly, you are likely to develop some symptoms of oestrogen withdrawal, such as hot flushes and night sweats. As HRT doesn't appear to be suiting you, however, it is important to talk to your doctor and discuss your problems, so he or she can advise you on what to do.

If you do decide to stop taking the HRT, the best alternative options include herbal extracts such as black cohosh (which can be combined with St John's Wort if you are not on other prescribed medication), sage leaf extracts and isoflavones.

One in three women who have had a total hysterectomy notice the loss of uterine sensations when making love, and seven out of 10 find it more difficult to achieve orgasm. Although there may be no difference in your sex drive at first, studies suggest that after a year women who have had a hysterectomy have significantly less orgasms than women with an intact womb. In a few cases, the opposite is true - some women find love-making more enjoyable and orgasm more intense after the operation, especially if they were previously in a lot of pain.
