Episiotomy worries

by Christine Hill

question
I am 32 weeks pregnant.
On the advice of my midwife I have been massaging my perineum in the hope of avoiding an episiotomy. I have noticed while doing so that the distance between my vagina and anus is only about 2.5-3.5cm.

This concerns me as when I was birth partner for my friend last year she had to have an episiotomy and the cut was 3cm to allow the insertion of a ventousse. I am worried in case I need an episiotomy myself and if they cut 3cm there would be no perineum left.

answer

I wonder if you might be fretting too much about the possibility of an episiotomy. Watching someone else giving birth can sometimes be frightening. It is very different when you are having a baby yourself.

I don’t think there is much difference in the length of the gap between the anus and vagina from one woman to another. In any case, when a baby is being delivered, the perineum is extremely stretched, so a measurement of 3 cm then would be quite different to that measurement under normal circumstances.

I’ve actually written about episiotomies – ‘Episiotomies: The Nasty Cut?’
this article should answer your questions and hopefully reassure you.
